LS 550 -- Application for Certificate of Eligibility to Employ Child Performers   This form must be completed by an employer prior to employing any Child Performer.

LS 551 -- Verification of Workers' Compensation/Disability Insurance Coverage   This form is to be completed by Payroll Services for applicants for Certificates of Eligibility to Employ Child Performers and submitted with the application along with forms C-105.2 and DB-120.1 from insurance carriers.

LS 284 -- Application for Meal Period of Less than Thirty Minutes   A meal period of less than 30 minutes but not less than 20 minutes will be permitted only in special or unusual cases after investigation and issuance of a special permit.

D-1 -- Dispensation – Defense Contractor   An employer engaged in the manufacture, production or distribution of articles or materials in fulfillment of defense contracts for the U.S. military and other related work may, pursuant to the provisions of the Defense Emergency Act of 1951, be granted a dispensation from certain requirements of the Labor Law.

LS 120 -- Application for Dispensation-Processors of Perishable Fruits and Vegetables   An employer engaged in processing perishable produce may be granted a dispensation from certain requirements of the Labor Law, pursuant to the provisions of the Defense Emergency Act of 1951.

Variation -- Day of Rest     An employer may petition the Commissioner of Labor for a variance from the requirements of Section 161 (One Day Rest in Seven) of the New York State Labor Law.

LS 390 -- Employment Agency Law   Full text of the law.

LS 355 -- Application for an Employment Agency License   Prior to the establishment of an employment agency, an Employment Agency License must be obtained. If the agency is to be conducted in New York State, but outside of the five boroughs of New York City, the License is issued by the Commissioner of Labor.  If the employment agency is to be conducted in the City of New York, the license is issued by the Commissioner of Consumer Affairs of such city. 

LS 355.1 -- Application for an Employment Agency License Renewal   Since Employment Agency Licenses automatically expire on May 1st of every even-numbered year, to remain valid each such license must be renewed biennially.

LS 355.2 -- Application for an Employment Agency Manager Permit   If the person responsible for direction and operation of the placement activities of an employment agency is not the individual (or a principal of the entity) applying for the Employment Agency License, then a separate Employment Manager Permit must be obtained.
