Deceased Claimants

Claim information for a deceased claimant may be requested by writing a letter to New York State Department of Labor, P.O. Box 15130, Albany, NY 12212. The letter should include:

1. Requestor’s name, address and telephone number
2. Requestor’s relation to the deceased
3. The deceased’s name and Social Security number
4. Information needed
5. A copy of the death certificate

Release of funds remaining in the Chase Direct Payment Card account of a deceased claimant, must be requested directly from JP Morgan Chase. The executor or administrator of the claimant's estate must contact Chase Customer Service toll-free at 1-877-221-1634 for instructions on how to submit the request.
